The COVID-19 pandemic has taken a toll on communities both near and far. With support from Colorado Access, Green Valley Ranch Medical Clinic & Urgent Care worked to make innovative changes to their clinic to continue to meet the needs of the diverse Green Valley Ranch community. From integrated care to a separate COVID-19/respiratory clinic, to future plans for COVID-19 vaccine distribution, learn more about their partnership with Colorado Access to serve the community at a time when it is needed most. Connecting the Dots - Social Determinants of Health
This new series will illuminate social factors that lead to health inequalities and disparities or help to decrease them. We hope this monthly series will support your existing efforts by providing resources and educational opportunities while exploring current trends and best practices. COVID-19 Vaccination Sites are Safe Spaces
for Immigrants

The Department of Homeland Security implemented a COVID-19-specific policy that designates COVID-19 vaccination sites as sensitive locations. This means that no federal immigration enforcement activities will take place at or near these sites. If you hear of any enforcement activities occurring, please contact the National Health Law Program here.
Multilingual COVID-19 Resources
The Centers for Medicare & Medicaid Services (CMS) has resources in eight languages available that outline how individuals and families can get the care they need during COVID-19 and provide information about health coverage plans. Learn more here.

The Delores Project
Our research and evaluation team recently completed an evaluation of our collaboration with The Delores Project, which shows that there were 1,877 successful care management interactions and 18 successful telehealth visits.
This evaluation shows innovation during a time of crisis (COVID-19), where we pivoted to provide ongoing support
and care to the residents at The Delores Project. This collaboration supported affordable care to those who qualify
for affordable housing for minority groups. Read more here.
